You can call (877) 577-6227 and a scheduler will set your patient up at the clinic closest to their home or work unless otherwise requested by the referring office.
If you are familiar with our Fax Referral you can click Online Patient Referral and give us a few pieces of information and then we will call the patient set up the appointment for you and then call you back to let you know the date it is scheduled for your records.*
You may also contact Dan Haskins if you are interested in utilizing our Fax Referral which is the same format as our online referral form except it is faxed. We will personally drop the forms off for you and answer any questions you may have about the process.*
